Accessing eSyM in MyChart

What is MyChart?​

MyChart is your own unique patient portal where you can manage your health information and communicate with your care team.

 

With MyChart you can:

  • Review test results, medications, immunization history, and more

  • Stay in touch with your physician

  • Manage your appointments

  • Upload health and fitness data

  • View and pay your bill

  • Access your family's health information

  • Report your symptoms via eSyM questionnaires!

How to Access MyChart

If you already have a MyChart account, download the MyChart mobile app below and simply add your organization.

If you do not have a MyChart account yet or have trouble accessing your account on the mobile app, please read additional information from your organization below to find out how to get started using MyChart today!

Baptist Memorial Hospital

Steps to activate your MyChart with an activation code:

  1. Go to mychart.baptistonecare.org and choose "Sign Up Now."

  2. Enter your 15 digit code, last 4 digits of social security number and date of birth. 

  3. Create a 6-20 character username, password, and security question.

  4. Enter your email address. (optional)

  5. Accept user agreement.

Steps to activate your MyChart without an activation code:

  1. Go to mychart.baptistonecare.org

  2. Click "Sign Up Now."

  3. Click "Sign Up Online." 

  4. Enter the requested information and click "Submit."

Dana-Farber Cancer Institute

You may have been emailed or text an activation code after a clinic visit.

Steps to activate your MyChart with an activation code:

  1. Go to mychart.partners.org and choose "Enroll Now."

  2. Enter your 15 digit code, last 4 digits of social security number and date of birth.

  4. Enter your email address.

  1. Go to mychart.partners.org

  3. Under "No Activation Code?" click "Enroll Now."

  4. Enter the requested information and click "Submit."

Dartmouth-Hitchcock Cancer Center

1. Go to mydh.org

2. Click "Create an Account"

3. Enter your birth date

4. Create a 6-20 character username, password, and security question.

5. Enter your email address.

6. Accept user agreement.

Lifespan Cancer Center

1. Go to my.lifespan.org

2. Click on the Sign Up Now link in the "New User?" box.

3. Enter your MyLifespan Activation Code. You will not need this code after you have completed the sign-up process. If you do not sign up before the expiration date, you must request a new code. 

4. Enter your Date of Birth as indicated and click Next. You will be taken to the next sign-up page. 

5. Create a MyLifespan username. Think of one that is secure and easy to remember. 

6. Create a MyLifespan password. You can change password at any time. 

7. Choose a security question, enter your answer, and click Next. This can be used to access MyLifespan if you forget your password. 

8. Select your communication preference. Enter a valid email address to receive email notifications when new information is available in MyLifespan. 

9. Click Sign In. You can now view your medical information.

Maine Medical Center

1. In your internet browser, go to mychart.mainehealth.org

2. Locate the New User section and click the "Get Started" button.

3. Enter the activation code from your After Visit Summary which you were given at your most recent office visit or click "Request Online"

4. Complete the prompts for information.
